Arlene Bel Zak of Florida, NY, entered into rest on Sunday, November 04, 2012 in ORMC, Middletown. She was 64.
The daughter of the late Jerry Simcik & Corinne Johnson Simcik, she was born on October 9, 1948 in Manhattan, NY.
The family would like to thank all of the staff at Elant and ORMC as well as her close friends and family that helped her during her illness.
She is survived by her brother, Jerry R. Simcik & nephew Christopher Simcik of Florida, NY.
Visitation will be held on Tuesday, November 6th from 4-6:30PM at T.S. Purta Funeral Home, 22 Glenmere Avenue, Florida, NY 10921.
Funeral service will be held on 6:30PM, Tuesday, November 6th at the funeral home.
Burial will be held privately in the Florida Cemetery.
Memorial contributions may be made to St. Jude Children's Research Hospital, 501 St. Jude Place, Memphis, TN 38105-1905 or to Warwick Valley Humane Society, PO Box 61, Warwick, NY 10990.
