Lisa Marie Kowalczik of Mt. Hope, NY, entered into rest on Sunday, December 15, 2013 at home. She was 44.
The daughter of Edward Dombal & Barbara Eagan Dombal, she was born on June 30, 1969 in Middletown, NY.
She is survived by her son, Alexander Kowalczik, daughter, Elly Kowalczik both of Mt. Hope; her parents, Edward & Barbara Dombal of Slate Hill; brother, Edward Dombal & wife Debbie of Slate Hill; brother, Greg Dombal of Guilford, NY; sister, Michele Donovan of Lachawaxen, PA; sister, JoAnne Dombal of Redwood City, CA; sister, Barbara Korn & husband Michael of Slate Hill; longtime companion, Joe Palazzo; friend, Jerry Kowalczik. She is further survived by several nieces, nephews, grandnieces and grandnephews.
Memorial visitation will be held on Friday, December 20th from 2-4 & 7-9pm at the T.S. Purta Funeral Home, 690 County Route# 1, Pine Island, NY 10969.
A memorial service will be held 10AM Saturday, December 21st at the funeral home. Burial of her cremains will be held at the Union Cemetery, Slate Hill, NY.
In lieu of flowers, memorial contributions may be made to St. Jude Children's Research Hospital, PO Box 1000, Dept. 142, Memphis, TN 38101-9908.
